CoQ10 Supplementation in PCOS: Improving Egg Quality and Pregnancy Outcomes

When you’re navigating fertility or PCOS, you often see long lists of supplements claiming to improve egg quality, hormone balance, or pregnancy outcomes. But when researchers ask, “Which nutrients really have solid human evidence?” the list gets much shorter.

In 2025, a systematic review and meta-analysis published in Reproductive BioMedicine Online tackled this question. Rather than looking at just one study, the researchers screened 1,810 papers and included only high-quality, controlled human studies. In the end, only five studies met the strict criteria - highlighting how limited strong research on fertility supplements actually is.

Across these studies, 66 different micronutrients and compounds were evaluated. Despite the large number tested, only Coenzyme Q10 (CoQ10) consistently showed meaningful benefits in humans.

What did CoQ10 actually improve?

1. Pregnancy rates (especially in PCOS)

CoQ10 significantly increased clinical pregnancy rates, particularly in women with PCOS:

  • Women with PCOS had a dramatic improvement in pregnancy rates when CoQ10 was added
  • Benefits were also seen in women with poor ovarian response
  • No meaningful benefit was seen in women over 35 without PCOS or poor ovarian response

This suggests CoQ10 works best when there is an underlying metabolic or egg-quality challenge, which is common in PCOS.

2. Egg quality and cellular energy

Laboratory studies showed that CoQ10:

  • Improved egg maturation
  • Reduced chromosomal errors (aneuploidy) in eggs
  • Was particularly effective in eggs from:
  • Women with PCOS-related dysfunction
  • Women with age-related egg quality decline

This is important because chromosomal errors are a major reason embryos fail to implant or miscarry.

3. Live birth and miscarriage outcomes

  • CoQ10 did not increase miscarriage rates
  • Live birth rates were modestly higher in women with poor ovarian response
  • Data was too limited to confirm strong effects on live birth across all groups

In fertility research, this often reflects small study sizes, not lack of biological effect.
